| Description |
Provides seed funding for the purpose of establishing or improving activities, methods, technologies, and processes related to the provision of community pharmacy-based patient care. The award will focus on a particular health condition for which a grant application and proposal must address to be eligible for consideration. |
| Amount |
Each grant will provide
$3,000 or $5,000 to support up to an 8-month project plus up to a $1,500 stipend to attend the NCPA
Annual Convention to participate in the award presentation. |

Applicants must be current NCPA members and currently licensed and engaged in independent
pharmacy practice. |
| Instructions |
- Applicants must submit a completed application form and project proposal.
- A representative must be available to travel to the NCPA Annual Convention in Philadelphia, PA
during the week of October 23 – 27, 2010 to participate in the award presentation.
- Recipients must submit to the NCPA Foundation a brief mid-term project report, a final project
report, and a final expense report. The final report must be made available for publication,
distribution, and presentation. The report should conform to standards for scientific and clinical
evaluations/studies.
