| Description |
The CT-ORH Rural Healthcare Initiatives Grant offers to assist CT rural community agencies/organizations, rural hospitals, rural clinics, and rural healthcare provider groups or organizations with specific initiatives that will positively impact access to quality healthcare for rural residents of CT.

| Amount |
Two to three grants will be awarded for rural health care initiatives. Total grant funds available are $ 34,000.00. Application budgets should reflect costs totaling between $ 10,000-24,000. |

| Eligibility |
Applicant must be a public or non-profit organization and must be located in a designated rural area as defined by the 2004 CT-ORH definition or demonstrate that a minimum of 75% of the proposed project activities are focused on serving CT rural residents.
Applicants may request funding for one initiative only. All applicant awardees must agree to share information and outcomes of their initiative. |
