Eligible applicants may apply for:
Tier 1 Funding:

New or expansion of a community primary care medical or dental clinic

$200,000 maximum award
-
25% of awarded funds must be matched
-
Project must be in a designated HPSA or MUA/P, or:
-
An applicant with an operational primary care clinic not located in a HPSA or MUA/P may apply if applicant can demonstrate that the primary care clinic serves either:
-
30% Low Income Patients (Medicaid, AdultBasic, Children's Health Insurance Program (CHIP), discounted/sliding fee, and/or no-pay), or;
-
30% of patients that currently reside within a HPSA or MUA/P
Tier 2 Funding:
-
The establishment of new Community Health Center serving a MUA/P lacking an existing Federally Qualified Health Center (FQHC) or Look Alike (FQHC-LA)

$500,000 maximum award
-
25% of awarded funds must be matched

Project must serve a designated MUA/P

A request for Applications (RFA) is issued annually. Applications may only be submitted when an RFA is issued. Next anticipated request for applications (RFA) fall of 2010. |
