Description	Seeks to recognize providers for quality patient care, improve MaineCare members' access to providers, establish a medical home, increase care continuity, strengthen the patient provider relationship, and reduce unnecessary expenditures and utilization.
Amount	Provides additional compensation to providers who deliver high quality health care to their patients and who rank above the 20th percentile for certain measures when compared to other physicians within their specialty.
Geographic Coverage	Maine
Eligibility	Only the following providers can enroll as Primary Care Case Management (PCCM) Primary Care Providers (PCP) Family and general practitioners (MD/DO) Internists Pediatricians Obstetricians/Gynecologists Physician extenders (Physician Assistants (PA); Nurse Practitioners: (NP, FNP, CFNP, CPNP)
